This Sports & Extracurricular Activity Waiver and Release of Liability ("Waiver") is executed by the undersigned parent or legal guardian ("Parent") on behalf of the minor student identified below, in connection with participation in athletic or extracurricular activities organized by __________ . I understand and acknowledge that participation in __________ carries inherent risks of physical injury, including but not limited to sprains, fractures, concussion, and in rare cases, catastrophic injury or death, and I voluntarily assume all such risks.

In consideration of being permitted to participate in __________, the undersigned, on behalf of themselves, the participant, and their respective heirs, executors, and assigns, hereby releases, waives, discharges, and covenants not to sue __________, its officers, employees, coaches, volunteers, and agents (collectively, the "Released Parties") from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant while participating in this activity, to the fullest extent permitted under the law of California.

The undersigned further agrees to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost arising out of the participant's participation, except where caused by the gross negligence or willful misconduct of the Released Parties, to the extent such exclusion is required by applicable law.

The undersigned authorizes __________ staff, coaches, or on-site medical personnel to secure and administer emergency medical treatment, including transport to a medical facility, for the participant in the event the undersigned cannot be reached, and agrees to be financially responsible for any costs of such treatment not covered by insurance.

The undersigned consents to __________ photographing or filming the participant during this activity and using such images or footage in school publications, websites, or promotional materials.

This agreement shall be governed by and construed in accordance with the laws of the State of California, without regard to its conflict of law principles. If any provision of this agreement is found to be unenforceable, the remaining provisions shall continue in full force and effect.

BY SIGNING BELOW, THE UNDERSIGNED REPRESENTS THAT THEY ARE THE PARENT OR LEGAL GUARDIAN OF THE MINOR PARTICIPANT WITH FULL AUTHORITY TO SIGN, HAS READ AND UNDERSTOOD THIS WAIVER IN ITS ENTIRETY, AND VOLUNTARILY AGREES TO ALL TERMS HEREIN ON THEIR OWN BEHALF AND ON BEHALF OF THE PARTICIPANT.
